We know that you care about your loved ones and want to stay informed with the most up-to-date information about our care community and the ongoing impact of COVID-19. Please know that we are committed to following the guidelines of CMS, the CDC, and our state officials in order to keep our everyone at our facility as safe as possible.

We are happy to report that visitation has resumed at Weirton Geriatric Center, Serra Manor, Grace Cottage, and Hope Cottage. It is critical for the safety of everyone that those who wish to visit understand and follow our guidelines for visitation. If you wish to schedule a visit with your loved one, please call during regular business hours (M-F, 8AM to 5PM). Please read below for our current visitation guidelines: (Updated June 2, 2021)

  • Serra Manor, Grace Cottage, and Hope Cottage will be open for visitation Sunday — Saturday 10AM to 7PM, by appointment. We prefer no visitation during meal time, unless you are coming to assist your loved one with their meal. Visits will be scheduled in the resident’s room if that is the case.
  • No more than 3 visitors per resident at a time. No more than 6 visitors per neighborhood at a time in designated areas. No age restrictions on visitors.
  • Visits can be booked for 60 minutes per visit. We prefer that visits be booked on the hour.
  • Visitors may book more than one visit per day, if space allows.
  • Outdoor visitation is preferred wherever possible, even when the resident and visitor(s) are fully vaccinated against COVID-19. Outdoor visits pose a lower risk of transmission.
  • Indoor visitation is allowed at all times for all residents (regardless of vaccination status), except for a few circumstances when visitation should be limited due to a high risk of COVID-19 transmission. These scenarios include limiting indoor visitation for:
    • Unvaccinated residents, if the nursing home’s COVID-19 county positivity rate is > 10% and < 70% of residents in the facility are fully vaccinated. 
    • Residents with confirmed COVID-19 infection, whether vaccinated or unvaccinated, until they have met the criteria to discontinue transmission-based precautions, or
    • Residents in quarantine, whether vaccinated or unvaccinated, until they have met criteria for release from quarantine.
  • Relaxed indoor visitation allows for close contact between visitors and residents, as long as the resident has been fully vaccinated at least two weeks prior to visit. The resident & visitor must wear a well-fitting face mask and perform hand-hygiene before and after the visit.
  • Compassionate care visits are permitted at all times, regardless of community prevalence, facility COVID-19 status, resident COVID-19 status and/or vaccination status.
  • Virtual visitation is also available for any residents who are COVID-19 positive and residents who are in isolation or quarantine precautions. These alternative methods include phone calls, video phone calls or window visits.
  • Core principles of COVID-19 infection prevention must be adhered to for staff, residents and visitors.

These core principles include:

    • Screening for all who enter the facility, which includes temperature checks, screening for signs and symptoms of COVID-19 or those who have had close contact with someone with COVID-19 infection in the prior 14 days (regardless of the visitors’ vaccination status).
    • Hand hygiene upon entering the facility and after your visit.
    • Face mask covering nose and mouth must be worn at all times while in the facility.
    • Social distancing at least 6 feet between persons unless the resident is fully vaccinated. They can choose to have close contact including touch with their visitors while wearing a well-fitting face mask and have used proper hand hygiene before and after the visit. Regardless, visitors should always physically distance for other residents and staff in the facility.
  • If the county spread increases or a facility outbreak occurs, the visitation policy may change.

Definitions:

Census – This number includes all residents who are currently under our care.

Positive – This is the total number of confirmed COVID-19 test results. Residents and employees are removed from this total if follow-up COVID-19 testing produces negative results.

New Onset of Respiratory Symptoms Within the Last 72 Hours – In addition to reporting confirmed COVID-19 cases, we are required to inform residents and their representatives where there are three or more residents or staff with new onset of respiratory symptoms occurring within 72 hours of each other.

Pending – This number includes residents and employees with symptoms who are currently being tested for COVID-19 whose results we are currently outstanding. Asymptomatic residents and employees who are being tested because they have been out of our facilities for an extended time or who may have come into contact with somebody who tested positive for COVID-19 are not included.

We are now in Phase Green, so limited visitation has resumed! It is as important as ever that you understand both how we keep our facility safe and what you can do to prevent the spread of COVID-19. Please see above for visitation details.

Facility Prevention Efforts:

Restrict Visitor Access: It is with heavy hearts that we must discontinue visitation until we are no longer in Phase Red. This is necessary for the continued safety of all our residents and staff.

PPE Supplies: We continue to prioritize access to bulk supplies of PPE and sanitation products to aid in the control of infectious disease.

Staff Training:  Our staff has been given special training with respect to best practices for disease prevention.

Alternative Visitation Options: We offer numerous ways of spending time with your loved one such as phone and video calls.

COVID-19 Detection and Testing: All employees are screened at the beginning of each shift for COVID-19 symptoms, including temperature checks and all employees must use proper PPE while in the facility. Furthermore, we screen residents daily for COVID-19 symptoms and proactively test residents showing symptoms based on CDC criteria.

What You Can Do to Help:

DO wash your hands regularly and practice good general hygiene.

DO continue to wear a protective mask when out in public.

DO continue to social distance and avoid places with large crowds.

DO self-quarantine if experiencing symptoms such as fever and cough.
